Job Description

Position Summary: Provides assessment services coordination and counseling to geriatric clients eligible for receiving multiâservices in the community and in general aides clients in managing life situations; e.g. budget medical care shelter recreational and educational activities social services and health programs.

Principal Responsibilities :

• Conduct intakes provide accurate information and referrals.
• Screen and assist clients with applications and re-certifications for entitlements and benefits
• Assess clients in their home hospital or other institutional setting as appropriate
• Advocate on behalf of clients and arrange for provision of services; develop implement and follow up on care plans
• Provide supportive counseling to clients as needed
• Maintain ongoing communication with client family members and other collaterals as appropriate
• Maintain complete and accurate case notes utilizing the programs data system; prepare required documentation in case file within organizational time frames
• Maintain and report on statistics according to Selfhelp standards utilizing the programs data system and other forms of record keeping as needed.
• Actively participate in supervision and staff meetings and share any emergent issues with supervisor in a timely manner
• Facilitate and assist at social recreational and other group activities and events
• Supervise social work students (if applicable)
• Adhere to organizations standards by completing annual mandatory trainings in a timely manner on topics including but not limited to Sexual Harassment Prevention Training IT Security Awareness Compliance OSHA HIPAA and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.
• Supports organizations mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
• Adheres to the organizations policy in regards to absenteeism and appearance and health and safety standards

Salary Range: $60000 - $62400 per year commensurate with experience

Job Competencies & Minimum Qualifications :

• Bachelors required; MSW preferred
• Working knowledge of Microsoft Office Suite and other technology
• Excellent communication and listening skills
• Excellent customer service skills with a focus on treating clients with respect and dignity
• Able to multi-task and work independently with a great attention to detail

Working Conditions/Physical Demand

• Business office environment with phone and computer use travel via public transportation. This position may have the ability to work from home 2-3 times per week.
