Day to Day:
The RN Care Manager can contribute to the assessment of health status through gathering and recording of assessment data, observing, and monitoring and reporting of changes in condition. Conduct interviews to understand cause and effect, gather or review health history for clinical symptoms, and determine health needs. The RN Care Manager collaborates with internal and external stakeholders on managing a successful transition from hospital to home. The RN Care manager in collaboration with the RN Care Manager is responsible on completing timely post discharge patient outreaches to identify, develop, implement, and assist on interventions for high-risk patients. The RN Care manager in supports and manages the post-discharge needs to reduce hospital readmission, improve patient care engagement, and improve patient experience. This is a remote position. Standard hours.
