Job Description

We're seeking a compassionate and results-oriented Registered Nurse (RN) to join our team as a Care Coordinator. In this role, you'll play a vital part in guiding injured employees towards a successful recovery.

What You'll Do:

Collaborate with a multidisciplinary team, including physicians, insurance adjusters, and other healthcare professionals, to develop and implement personalized care plans.
Advocate for the best possible outcomes for injured employees, ensuring they receive timely and appropriate medical care.
Coordinate all aspects of medical care, including scheduling appointments, arranging transportation, and securing necessary equipment and home modifications.
Conduct regular patient assessments and monitor progress throughout the recovery journey.
Build strong relationships with patients and their families, providing support and guidance every step of the way.

What You'll Need:

Active Arizona Registered Nurse (RN) license.
Valid Arizona Driver's License and Auto Insurance.
Proficiency in Microsoft Word and Excel.
Bilingual (Spanish) is required.
1+ years of experience in nursing (preferred).
1+ years of experience in case management (preferred).
Excellent communication, interpersonal, and organizational skills.
Strong patient advocacy and problem-solving abilities.
