Job Description

SUMMARY OF DUTIES AND RESPONSIBILITIES:

JOB SUMMARY: Under the direction of the Head Nurse and Medical Director of the Clinic will provide a wide range of education for patients in developing skills and knowledge to prevent sexually transmitted infections (STI), including HIV, and support treatment for individuals and their partners diagnosed with an STI.

RESPONSIBILITIES:
•This position requires working about 20% of the time during late-night or evening shifts at a mobile sexual health clinic.
•Provides Patient Navigation support.
•Follow up with patients seen on the mobile unit and make appointments at Gouverneur or another site convenient to patient.
•Following guidelines provided by the Medical Directors of the Departments of Medicine provides patient counseling on:
•Familiarity with sexually transmitted infections (STls), point-of-care (POC) testing, and fundamental healthcare insurance information for patients.
•Collects data and initiates surveys, as requested, for particular projects.
•Performs telephone follow-up with patients to support self-management activities.
•Performs health related procedures including: vital signs, pulse oximetry, peak flow, collecting medical samples for diagnostic purposes; performs point of care testing including: rapid HIV testing and pre and post counseling, urine dip slick, rapid strep A testing, pregnancy testing, etc.
•Conforms to policies governing uniform code, attendance and punctuality.
•Follows all infection control guidelines and protocols.
•Conforms to policies governing patient rights and confidentiality of patient information.
•Performs clerical functions including, registering patients, scheduling and rescheduling appointments, verifying insurance via RTE when necessary, opening visits, discharging patients, and documenting in the electronic medical record.
•Handling of Referrals via Epic EMR system, reconciles patient appointments including outreach as needed and documenting in the electronic medical record
•Participates in Outreach events and health fairs, both on and off the premises when needed.
•Assists in emergency situations under the supervision of the professional staff.
•Attends required in-service and on-going training and acquires skills that may be required in a changing healthcare environment.
•Seeks assistance and guidance in performance of duties, as necessary.
•Assists in forms filling as liaison between patients and provider.

Flexible work schedule Is a must as this position requires working evenings, including weekend evenings, for mobile sexual health clinic sessions

•Assist with special projects as assigned

Minimum Qualifications
1. A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education or in health, social or biological sciences; or
2. A baccalaureate degree from an accredited college, and six months of full-time satisfactory experience in a health promotion or disease intervention prevention program, performing one or more of the following: interviewing, conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data; or
3. A four-year high school diploma or its educational equivalent, and four years of full-time satisfactory experience as described in "2" above; or
4. Education and/or experience equivalent to " 1 ", "2", or "3" above. Undergraduate college credit can be substituted for experience on the basis of 30 semester credits from an accredited college for one year of full-time experience. However, all candidates must have a four-year high school diploma or its educational equivalent, and either twelve semester credits as described in "1" above or six months of experience as described in "2'' above.
5. One year of experience as a Public Health Adviser, Assignment Level I, or at least one additional year of experience as described in "2" above.

Additional Requirements:
1. For certain assignments, satisfactory completion of training in phlebotomy or other necessary procedures is required. Certification in HIV counseling may be required for certain other assignments.
2. A valid New York State Motor Vehicle Driver license may be required for certain assignments. If required, this license must be maintained for the duration of employment.
3. Public Health Advisers are required to have an annual health assessment.

EDUCATIONAL LEVEL:

Associate Degree required; Bachelor of Science Degree preferred

KNOWLEDGEABLE IN:

The ideal candidate will have:
•Completion of basic course work in medical terminology, anatomy and physiology, patient Instruction and communication within the medical setting.
•Certification in CPR, phlebotomy, ECGs, and Point of Care testing.

•Ability to exercise tact in all interpersonal contacts and always maintain confidentiality
•Bilingual language fluency is helpful

YEARS OF EXPERIENCE:
•Minimum of 3-5 years of experience in community sexual health campaigns and/or work within healthcare settings providing sexual health services
