Description

Eddy Visiting Nurse & Rehab Association in Troy, NY has an opportunity for a full-time RN to triage calls after hours to assist patients and their caregivers with issues or questions!

Calls are handled by our answering service. Those requiring to speak with a nurse are called out to the on-call Triage Nurse. If patient needs a visit triage nurse notifies the on call nurse.

The RN Triage Nurse Will (but Not Limited To)

• Triage calls, assisting patients/families telephonically if possible
• Determines if oncall visit RN needs to see patient at home, and contacts RN on call to share information regarding patient needs
• Coordinates with RN Supervisors, clinicians and schedulers across the agency to prioritize patient needs
• Participate in Team educational meetings

Minimum Requirements

• NYS RN licensure
• 2 + years of experience as a RN in home care or acute care setting
• Valid driver license and reliable vehicle

Preferred Requirements

• 4+ year of home care experience
• Experience managing chronically ill patients (heart failure, COPD, diabetes, etc)
• Strong chronic care management
• Excellent computer skills and can work with multiple computer applications
• High level of communication skills to work with a diverse team across different departments
• Multitask and can accurately prioritize in a very fast pace environment
• Maintain the highest level of customer service while completing day to day duties and responsibilities
• Excellent critical thinking skills to problem solve Excellent time management and organizational skills

Pay Range: $33.50 - $46.95

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
