Sentara Health is hiring for a Nurse Case Manager/Private Duty Nurse/RN- Tidewater, VA!
Status: Full-Time, permanent position (40 hours)
Standard working hours: 8 am to 5 pm EST, M-F
Location: Remote but will conduct home visit in members' homes. Need to reside in Tidewater, VA (Norfolk, Virginia Beach, Suffolk, Portsmouth and Chesapeake, VA).
Nurse Case Managers serving Members in High Intensity Care Management with the following conditions:
Members receiving private duty nursing services
Life-sustaining Ventilator dependent members
Vulnerable infant members, including those diagnosed with neo natal abstinence syndrome, classified substance-exposed, or admitted at NICU Level 3
Job qualifications:
Min 3 years Nursing experience REQUIRED
Registered Nurse/RN License in Virginia/Multi-state/Compact REQUIRED
For Integrated Care Management departments, specialty certification REQUIRED within one year of eligibility (ACM, CCM, CCCTM or RN-BC). For other service lines, certification based on specialty area required within one year of eligibility.
Preferred qualifications:
3 years Case Management experience preferred.
1 year working directly with members who meet the conditions listed below:
a. Members receiving private duty nursing services
b. Life-sustaining Ventilator dependent members
c. Vulnerable infant members, including those diagnosed with neo natal abstinence syndrome, classified substance-exposed, or admitted at NICU Level 3
N-2YR - RN-Associate's Degree
N-4YR - RN-Bachelor's Level Degree
N-3YR - RN-Diploma (Non-degree)
N-DN - RN-Doctorate Level Degree
N-6YR - RN-Master's Level Degree
