Job Title: Locum Dentist (Eureka, CA)

• Location: Eureka, CA
• Rate: $1100/day
• Schedule: Monday to Friday, 8 AM - 5 PM
• Patient Mix: 50% Kids / 50% Adults
• Duration: 3 months minimum, with the option to extend (no exceptions)

Key Responsibilities:

• Provide comprehensive dental care to a mixed patient population (children and adults).
• Ensure timely chart closure within 72 hours after each patient visit.
• Follow all clinic and procedural protocols.

Qualifications:

• Active DEA license required before the start date.
• Must be willing to commit to a minimum of 3 months.

Credentialing Requirements:

• Credentialing process typically takes 30-60 days (six weeks for timely submission of application and documents).
• Required immunizations (must be completed prior to start):
• 2022-2023 Flu Vaccine
• TB Status/X-Ray Results (must be within 90 days before start date)
• Positive MMR Titer or Complete Immunization Series
• Positive Hep B Titer or Complete Immunization Series
• Positive Varicella Zoster Titer or Complete Immunization Series
• Tdap Immunization (within the last 10 years)
