Summary:

Join a growing senior-focused neighborhood-centric Primary Care provider as a Community Health Worker at their South Austin clinic! The role is Direct Hire and will be fully onsite M-F 8-5pm .

Position Overview :

In this role, you will act as a bridge between patients and their primary care provider. Providing culturally sensitive services to improve access to care and overall health outcomes. Seeking a teammate to guide patients to resources, advocate for their needs, and promote healthy behaviors. Position may also provide direct services like health education and basic screenings.

Position Day to Day:

1. Provides comprehensive care coordination to an assigned patient caseload.
2. Works collaboratively with patients, family, caregivers, healthcare providers, and external partners, to meet complex social needs.
3. Promotes a collaborative process and communication between all health care team members, internal multidisciplinary teams, inclusive patients/clients, families, and caregivers to ensure the process of integrated care services are targeted, appropriate, and beneficial.
4. Intervenes with patients and families regarding emotional, social, and financial consequences of illness and/or disability.
5. Conducts in-person visits to the patient’s homes, as needed, per the Home Safety Measures Policy.
6. Accesses and mobilizes family/community resources to meet social care needs.
7. Documents all interventions in the patient medical record both timely and accurately including all elements of clinic visits, in home, telephonic engagement, or texting.

Qualifications:

• Bilingual/Bicultural (English and Spanish)
• Community Health work experience, ideally 2+ years
• Community Health Worker Certification
